  • IMPORTANT: We note the name of the person who drops off your dog for treatment in your file on each visit. We will not release your dog to anyone other than the person who dropped him/her off unless they know your confidential password. It is your responsibility to keep your password current among the people who are authorized to pick up your dog and to notify us if you change your password. We obviously have no way of knowing about any changes you may have in relationships that could change the list of people who may, or may not, have your permission to pick up your dog.

  • Accuracy and Disclosure:

    Owner acknowledges and agrees that all the information provided to Barks and Medication is complete and accurate to the best of their knowledge. Owner certifies that Pet is in good health, up to date on required vaccinations (Rabies, Bordatella, and DHLPP – unless otherwise noted in writing), has not been sick during the last 30 days and is free from any communicable diseases. Owner agrees to complete and turn in up-to-date veterinary records to include in Barks and Medication Pet file and further agrees to authorize his/her former Vet to release any information about the Pet Barks and Medication may request. Owner agrees to maintain regular flea, tick and heartworm prevention as long as Pet is a client of Barks and Medication. Owner acknowledges and agrees that Pet is not aggressive and does not have threatening behavioral problems. Owner certifies that Pet has never harmed itself, another pet or any person. Owner understands, and acknowledges that each time Pet is brought in to Barks and Medication, Owner is recertifying that the Pet in in good health and has not been exposed to any communicable diseases of any kind within the last 30 days and if that is not the case, Owner will inform Barks and Medication of any illness or exposure to any communicable disease AT THE TIME THE APPOINTMENT IS MADE AND BEFORE PET ENTERS BARKS AND MEDICATIONS’ BUILDING. Owner agrees that if fleas or ticks are spotted on Pet at any time during their stay, Barks and Medication may administer a flea and tick bath, or oral medication, at the Owner’s expense.

    Barks and Medication reserves the right to refuse service to any Pet for any reason, at any time.

    Release of Liability:

    Owner acknowledges that Barks and Medication makes every effort to keep every dog and every human safe at all times in and around the facility, and that the concept and construction design of Barks and Medication allows dogs to socially interact with other dogs and humans during each visit. Owner releases Barks and Medication, its employees, and any agent associated with Barks and Medication from any liability for losses or damages of any kind suffered by Owner, the Pet, or anyone picking up or dropping off the Pet, from any injury, behavioral problem, health problem, and/or illness of the Pet, including without limitation, death.

    Owner voluntarily accepts these risks, and fully releases Barks and Medication, its employees, and any agent associated with Barks and Medication from any and all claims arising out of injury or damage in any way related from associating with Barks and Medication. Owner agrees and understand that dogs are unpredictable animals and Owner will be responsible for any and all costs associated with any kind of injury to their Pet, including but not limited to veterinary bills. In the event that Barks and Medication employees transport Pet to or from facility, Owner releases Barks and Medication from all liability in the event of an accident or injury during transportation. Owner agrees that regardless of any statement or agreement made, or implied herein, under no circumstance will Barks and Medication’s liability exceed the lesser of the current value of a Pet of the same breed, or a lump sum of $200 per Pet.

    Acceptance of Risk:

    Barks and Medication strives to maintain a safe and healthy environment for our guests, but Owner fully understand that animals are unpredictable and there are potential risks involved with interactions between dogs and humans, as well as dogs and other dogs, which may result in property damage or bodily damage, including, but not limited to illness, disability, or death to dog or human. Owner fully accepts and assumes all risks and responsibilities for all risks, including but not limited to death, veterinarian expenses, losses, damages, any cost, or any form of sickness, including but not limited to communicable disease contracted by Pet at Barks and Medication.

    Veterinarian Liability and Release:

    Owner acknowledges and agrees that Barks and Medication reserves the right to provide medical treatment for Pet if Barks and Medication believes in any way that pet may be ill, injured, or needs medication attention beyond the scope of the medical attention originally envisioned at the time of the visit. Barks and Medication will attempt to contact the Owner and/or the emergency contact for pet, but is hereby granted the right to provide Veterinary care in any emergency situation regardless of whether or not contact was made with the owner or emergency contact. Owner grants Barks and Medication employees(s) full power of decision making involving medical treatment and Owner will be fully responsible for all costs associated with the transportation to the veterinarian, and any bills associated with seeking help for Pet.

    Abandoned Dogs:

    No Pet shall be left at Barks and Medication past scheduled timing of visit without the agreement of the Owner and Barks and Medication. If Pet is left at Barks and Medication for more than seven days past scheduled pick up date without hearing from Owner or any person on approved pick up list, Barks and Medication will become the legal guardian over Pet. Barks and Medication reserves the right to seek a new home for Pet after seven days of abandonment. Owner fully acknowledges and agrees that if Pet is abandoned at Barks and Medication, Owner may not be able to retrieve Pet and cannot take any recourse against Barks and Medication.

    Payment:

    All payment is due at or before pick up. In some instances, payment, or a portion therof, may be required before services are rendered or begun. Owner agrees that Pet may not leave the facility until all charges due are paid by Owner or payment arrangement is agreed upon by both parties. Owner agrees to pay any collections costs and the cost of a returned check or debit charge. Owner agrees to pay any late fees associated with late or emergency pick-ups.

    Owner acknowledges that the terms “Pet” or “the Pet” in the use of this contract are considered one and the same and either or both refer to any and all pets under the ownership of “Owner” or “the Owner” both considered one and the same in this contract. Owder acknowledges that he/she has read, understands and agrees to all of the above.
